Registration Requirements
- Form 2: Acknowledgement of parents’/guardians’ reading and understanding of the FiSCA prospectus (downloadable from our website).
- The FiSCA application form (downloadable from our website).
- Learner information form (downloadable from our website).
- Health/medical information form from the doctor or nurse.
- Proof of child’s immunizations.
- Permission to administer medications together with the medicine and the accompanying instructions (if applicable).
- Proof of developmental screening information showing the date it was done.
- P200 non-refundable application handling fee together with the term’s school fees payment.
